Evaluation forms from February 10th Human Error Prevention Seminar

Attached are the evaluation forms and attendance list from last Friday’s seminar.  I reviewed them and it appears that most attendees rated the seminar good to excellent; and no strong negative comments were received.  Some of the attendees asked for a root cause analysis seminar, and we will work on that.

Dear Russ:
Thanks for the 23 evaluation forms….

On a scale of 1-5, the latter being excellent, the evaluations for  presentation attributes under my control averaged as follows:

Knowledge of instructor – 4.83;
Use of examples – 4.74;
Teaching style 4.52;
Organization of material – 4.48;
Encouragement of questions – 4.09;

The latter score suffered because lunch took almost 1 ¾ hours, reducing the time for questions ….  Never-the-less, the first four scores are very good and the last score is certainly acceptable.
